Our 2025 Year in Review
Changent unites two of the nation’s leading two-generation programs, Child First and Nurse-Family Partnership, under a unified vision of family well-being.
In 2025, our comprehensive approach, made possible by the commitment of our partners and supporters, delivered effective, coordinated care for families, resulting in measurable, positive outcomes for families and communities nationwide.
Dear Partners and Supporters,
2025 marked a defining moment for Changent. With the launch of our new brand name, we brought Child First and Nurse-Family Partnership together under one organization with one vision for family well-being. Changent means that a united national network is better positioned to meet the needs of families and communities by creating pathways for interconnected support.
This year placed real strain on families, communities and the systems meant to support them. Families faced rising costs, mental health challenges, growing barriers to healthcare and more. Providers navigated increasing workforce pressures, while funding for many social services was reduced or eliminated altogether. When systems falter, families bear the consequences first. That’s why Changent’s work is more important than ever.
Amid these challenges, we stayed focused on what matters most. Through ongoing training, program implementation support and technical assistance, we supported our local partners as they navigated a complex and shifting landscape, ensuring families continued receiving the personal support of their Nurse-Family Partnership nurse or Child First care team at a time when they needed it most.
Our focus led to meaningful outcomes. Together, we served over 59,000 families in 2025. Families entered these programs during pivotal moments – pregnancy, early childhood and times of acute stress – and saw measurable results: healthier pregnancies, stronger parent-child relationships, and greater stability for families. These outcomes reflect not just our reach, but the consistency and quality of support delivered across communities.
Changent also plays a broader role in sustaining this work. In 2025, our efforts generated $427 million in public funding for Child First and Nurse-Family Partnership, ensuring these services remain available to families who need them.
Looking ahead, we know that challenges persist. We are focused on protecting access to programs like Child First and Nurse-Family Partnership, strengthening coordination across systems and supporting our partners through continued uncertainty – always with families at the center.

In 2025, Changent expanded our footprint through 37 implementing partner organizations, adding 3 new partners and expanding services through 34 existing partners to deliver Child First and Nurse-Family Partnership to even more families and communities.
New Partner spotlight:
Puerto Rico
In 2025, Changent continued to expand our reach, bringing our proven programs to new communities and territories, including the first-ever Nurse-Family Partnership program in Puerto Rico – Goodwill de Puerto Rico. Thanks to a groundbreaking initiative managed in partnership with Goodwill Legacy Group and the steadfast support of a Community Advisory Board of local leaders, families on the island now have access to critical home visiting services, supporting healthier pregnancies and long-term family well-being.
Goodwill de Puerto Rico launched Nurse-Family Partnership in March 2025. Changent was proud to partner with Goodwill Legacy Group to support program start-up and deliver clinical training to prepare to serve Puerto Rican families.

Changent is dedicated to improving health outcomes by supporting and scaling two national evidence-based programs – Child First and Nurse-Family Partnership. Under the Changent umbrella, each program continues to operate under its trusted name, building on decades of proven outcomes for families and backed by Changent’s infrastructure of support.
In 2025, both Child First and Nurse-Family Partnership continued delivering meaningful, measurable impact for families facing significant barriers to health. While serving families with increasingly complex needs, both programs delivered strong outcomes that spanned maternal and child health, mental health, child development, parent-child relationships and family stability. Read on to learn more about each of our programs’ outcomes in 2025.
Child First, a program of Changent, is a home-visiting program that connects families with a mental health clinician and care coordinator who work together to provide comprehensive support to caregivers and their children. The program helps caregivers strengthen their relationship with their child, address the effects of trauma and stress, and connect with community resources—promoting long-term family well-being.

Child First Caregiver and Changent Parent Ambassador
Nurse-Family Partnership® (NFP), a program of Changent, pairs expectant parents and families facing complex challenges with a specially educated nurse home visitor who provides guidance, education and support during pregnancy and the earliest years of a child’s life. Through regular home visits, nurses partner with families to navigate pregnancy, build confidence in parenting, and create a healthy foundation for their child’s future.

Nurse-Family Partnership Spotlight: Dionna & Jess
Nurse-Family Partnership nurse Dionna Holt, R.N. supported first-time mom Jess through her pregnancy and earliest days of parenthood—a journey featured in PBS documentary American Delivery, which highlights the U.S. maternal mortality crisis and points to NFP as a trusted, community-based prevention program.
Building a Stronger System of Care
In 2025, Changent extended our work beyond delivery of Child First and Nurse-Family Partnership. As we’ve embraced our new identity as Changent, we are strengthening how we operate as an organization and how we contribute to the broader systems that impact families’ health and well-being.
That work starts internally – improving how we support our partners and deliver services – but it extends beyond. We are advocating for policies that deeply impact the lives of families, expanding access through program innovations, and collaborating with peers to build a stronger, more connected system of care.
Advocacy in Action
Changent plays an active role in shaping policies that impact our partners, clinical workforce, and the families and communities we serve. In 2025, Changent used our organizational voice to advocate at the state and federal levels on issues from safeguarding the clinical workforce that powers Changent’s impact to rejecting Medicaid funding cuts. When it became clear Medicaid work requirements would become law, Changent worked to ensure exemptions for parents and other caregivers of young children.
We also partnered with policymakers and community leaders to secure and protect public funding for evidence-based home visiting – and to maintain bipartisan support for the Maternal, Infant, and Early Childhood Home Visiting (MIECHV) program. In the face of immense funding uncertainty for social service programs, this work helped ensure that Child First and Nurse-Family Partnership providers could continue delivering services – and that families could continue to access the support they need without disruption.

Advocacy spotlight:
Parent Ambassadors
In 2025, Changent welcomed its fifth, and largest, cohort of Parent Ambassadors – a group of former participants in Child First and Nurse-Family Partnership who serve as trusted spokespeople and advocates for Changent. Drawing on their lived experience, Parent Ambassadors receive leadership training and advocacy support to engage with policymakers, share their stories publicly, and advocate for policies at the federal, state and local levels that improve the health and well-being of families. While the program is a volunteer opportunity, participants receive a stipend and ongoing support to grow as leaders and advocates.
The current cohort includes 12 Parent Ambassadors representing both Child First and Nurse-Family Partnership – a significant expansion of the program to drive deeper impact across the country.
Innovation in Action
Changent continues to expand access to care through NFP Expanded Eligibility, an initiative that broadens who can access Nurse-Family Partnership services beyond first-time parents enrolling early in pregnancy. This expansion includes expectant parents with a child(ren) and those enrolling later in pregnancy, beyond the 28th week. By reaching a broader population of families facing the steepest barriers to health and well-being, Changent is better meeting the needs of families and communities nationwide.

Leading the Field
Changent plays an active role in the home visiting, early childhood, and maternal, child, and mental health fields– collaborating with our peers and leading the way toward a more collaborative, connected system.
Leading the Way in Evidence-Based Home Visiting
Both Child First and Nurse-Family Partnership were featured in the 2025 Home Visiting Yearbook, a national resource on early childhood home visiting produced by the National Home Visiting Resource Center. Together, Changent provided nearly 20% of all evidence-based home visits across the country in the prior year, reflecting our continued scale and impact. Changent continues to work alongside other home visiting models to strengthen coordination and ensure that every family has access to the right support at the right time.
Advancing a Holistic Vision of Family Mental Health
In December, Changent leaders Rebecca Parilla, National Clinical Director of Child First, and Sharon Sprinkle, Senior Manager of Nursing Practice were recognized as part of the 2025 Care Guild, which honors leaders who are advancing a bold, family-centered vision for mental health— one grounded in the understanding that none of us gets healthy, or stays healthy, alone.
Financials
Changent leverages resources — from philanthropic contributions, investment income and earned income — to support communities and families across the country. Every dollar raised is needed and valued as we use these resources to advance Changent’s mission and invest in our program network. We are deeply grateful to our donors and supporters who fund our charitable mission.
Net Change = ($740,000)
Changent recognizes philanthropic support for financial purposes in the year in which a commitment is made, regardless of when funds are received or when expenditures are incurred. Net assets, therefore, may increase in a year when commitments are recognized and decrease in later years when associated funds are spent. Adequate net assets also ensure the organization has sufficient operating reserves to weather any future financial unpredictability.
