Positions available servicing Bergen or Passaic County
Step into a pivotal role as a Nurse Home Visitor, where you’ll provide life-changing support to first-time parents and their families through the Nurse-Family Partnership (NFP) program.
In this role, you’ll deliver exceptional nursing care, offering guidance and encouragement from early pregnancy through a child’s second birthday. Through personalized home visits, you’ll empower families, ensuring they have the knowledge, resources, and confidence to build a healthy future.
ESSENTIAL DUTIES:
Provide in-person home visitation to eligible families and carry a caseload of up to 25 clients in accordance with the NFP Model Elements, and guidelines.
Complete NFP education and nurse-home visitor competency requirements, including PIPE: Partners in Parenting Education, and ongoing professional development and webinar training.
Develop therapeutic relationships and utilize concepts of reflection and motivational interviewing with families in a home visiting environment.
Follow nursing process in implementing NFP into nursing practice.
Assess the physical, emotional, social, and environmental needs of parents and their families as they relate to the NFP domains.
Assist parents and families in establishing goals and outcomes; provides education, support, and referral resources in attaining goals; evaluates clients’ progress and plans home visits in accordance with clients’ goals and NFP outcomes.
Consult and collaborate with other professionals involved in providing services to families.
Meet with the NFP Nurse Supervisor weekly for clinical supervision and schedule joint home-visit every four months.
Attend team meetings, case conferences, and community events.
Maintain HIPAA confidentiality standards and NJ nursing process and standards, code of ethics.
Develop and maintain community relationships to support client referrals.
Handle other assignments as requested.
JOB REQUIREMENTS:
Associate Degree in Nursing (ADN) or Associate of Science in Nursing (ASN) required. Bachelor of Science in Nursing (BSN) preferred
Current New Jersey RN license (in good standing) required.
BLS certification required.
Bilingual English/Spanish required.
Two years’ experience in maternal/child health, public health, home visiting, or mental/behavioral nursing preferred
Excellent written and verbal communication skills.
Ability to work independently and manage client visit schedules
Strong organizational skills and ability to handle multiple priorities.
Basic computer skills and proficiency with Microsoft 365
Valid driver’s license and reliable transportation required for regional travel.
