Job Description:
Essential member of Nurse Family Partnership (NFP), an evidence-based, nurse home visiting program that improves the health, well-being and self-sufficiency of low-income, first-time parents and their children. Provides comprehensive community health nursing services to women and their families eligible for the NFP Program, maintains the highest standards in clinical nursing practice, adheres to the NFP model, and to policies, procedures, guidelines and standards of NFP and of the Prisma Health System. Self-directed and accountable for the coordination of care of assigned cases.
