Job Description:
Panhandle Health District is seeking a compassionate and dedicated Registered Nurse to join our Nurse-Family Partnership (NFP) Home Visiting Program. Job Summary:
The Registered Nurse for the Nurse-Family Partnership (NFP) Home Visiting Program at Panhandle Health District provides comprehensive home visitation services to eligible pregnant women and their families. This role requires the RN to uphold the highest standards of clinical nursing practice, ensure strict adherence to the NFP model and Visit-to-Visit Guidelines, and follow all Panhandle Health District policies and procedures. Additionally, the RN plays a key role in promoting public awareness of the NFP program within the community.
Duties/Responsibilities:
Adhere to the nursing process in implementing NFP into nursing practice.
Perform home visitation in accordance with the NFP model and the NFP Visit-to-Visit guidelines.
Carry a caseload of up to 25 clients (based on FTE).
Assess NFP clients in the areas of physical, emotional, social, and environmental challenges as they relate to the NFP program domains.
Assist NFP clients in establishing goals and provide education, support, and referral sources to help clients attain their goals.
Develop therapeutic relationships with NFP clients utilizing the concepts of reflection and motivational interviewing while respecting professional boundaries.
Obtain appropriate “Informed Consent” forms, ensure client confidentiality, and maintain client health records. Follows established PHD HIPAA/HITECH policies and procedures as well as State and Federal laws that govern the release of health care information.
Complete all required NFP education (reading, online Tracker modules, and in-person sessions).
Engage in professional development to meet NFP nurse home visitor competency requirements and assume responsibility for further professional development as educational opportunities are made available.
Perform public health emergency response duties as assigned and consistent with job classification and training provided, in response to threats to the public’s health.
Performs other duties as assigned, consistent with job classification.
Qualifications and Proficiencies:
Bachelor’s degree in nursing with a current nursing license.
Complete all required NFP education for nurse home visitors.
Meet competency standards for NFP nurse home visitors.
Ability to become CPR Certified within 60 days of employment.
Reflective listening and motivational interviewing.
Standards of nursing practice familiarity.
Clinical and program organizational skills.
Group and individual communication skills.
Knowledge of the core functions and essential services of public health.
Knowledge of current public health principles and processes.
Understand processes to counsel patients regarding health.
Comfortable with promoting community outreach efforts.
Good knowledge of Microsoft Office products.
Valid driver’s license (Idaho or Washington) or ability to obtain one if relocating.
National Incident Management System (NIMS)/Incident Command System (IS)100, 700 within one (1) month of employment.
Physical Requirements:
Due to the need for all Health Department personnel to respond to public health emergencies, the employee must be assessed for his/her ability to meet the physical demands of performing the following activities:
Engage in the following movements: climbing, balancing, stooping, kneeling, crouching, reaching, standing, walking, lifting, fingering, grasping, feeling, and writing and repetitive motions.
To move objects, exert up to 30 pounds of force occasionally and/or a negligible amount of force constantly.
Hearing ability sufficient to communicate effectively in person and over the phone.
Visual ability must be sufficient to read typewritten documents, computer screens, and drive a car
