Job Description:

This Public Health Nursing position will carry out the programmatic requirements of the Nurse-Family Partnership nurse home visiting program. Provide home visits to low-income women and their families, weekly to every other week. Adhere to the nursing process, and the NFP model of home visitation. Manage a caseload of approximately 21-25 clients. Support policies, procedures, NFP Visit to Visit Guidelines and standards of NFP and the Saint Louis County Department of Public Health. Participate in community coalition in support of NFP. and promote public awareness of NFP. Complete all required NFP Initial Education. Develop therapeutic relationships, utilizes concepts of trauma-informed care and motivational interviewing with women and their families in a home visiting environment. Perform home visiting in accordance with the NFP model and NFP Visit-to-Visit Guidelines. Assess physical, emotional, social and environmental strengths and challenges of women and their families. Provide education, support and resources to assist women and their families in attaining their goals. Participate in ongoing professional development (examples include: mental health, intimate partner violence, opioid use, health equity & reproductive health) to enhance their practice. Participate in weekly NFP team meetings, case conferences, and reflective supervision. Perform such additional duties as required.

The 3 year Children’s Service Fund grant will end on 6-30-26.

Examples of Duties:
Plans, organizes and delivers health care to clients and their families by assessing health status, determining needs, and providing resource referrals.
Promotes healthy client lifestyles through guidance and instruction regarding nutrition, child development, and related health issues.
Manages a client caseload which includes normal and high risk prenatal and infant clients, chronic disease clients, and family planning clients.
Prioritizes home visit referrals utilizing good organizational skills.
Initiates nursing assessments and develops appropriate ongoing nursing care plans, including short and long term goals.
Collaborates with other health care providers to assure delivery of required or needed services.
Interviews clients in the home to obtain health histories.
Evaluates the health status of clients by doing physical assessments.
Counsels clients and their families as indicated.
Evaluates developmental status of infants and children and instructs care giver in appropriate interventions and resources.
Educates clients and their families according to teaching and learning principles at client’s level of understanding.
Documents in a timely fashion assessments, interventions, evaluations, outcomes, and referrals on required forms.
Evaluates progress toward achievement of optimal health and modifies plan as indicated to facilitate attainment of goal.
Networks with providers to transmit knowledge to clients and staff.
Collaborates with other Public Health Nurses to address ongoing community health issues.
Performs related work as required.
