TITLE: Nurse Home Visitor – Nurse Family Partnership
REPORTS TO: NFP Supervisor
STATUS: FULL TIME EQUIVALENT (30 – 40 hours per week)

SUMMARY
The Nurse Family Partnership (NFP) nurse home visitor is responsible for providing comprehensive community health nursing services to women and their families eligible for the NFP program. The nurse home visitor is responsible for maintaining the highest standards in clinical practice and adherence to the NFP model. The nurse home visitor is responsible for adherence to policies, procedures, guidelines and standards of NFP and the lead agency.

BASIC JOB FUNCTIONS:
– Provides home visits to women and their families eligible for the SHNFP program.
– Adheres to the NFP model of home visitation.
– Carries a caseload of at least 21 – 25 clients per full-time home visitor and/or carries a caseload that results in an average of 32 plus visits per month.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:

  • Attends and completes all required NFP education.
  • Successfully achieves reliability with DANCE.
  • Develops therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home-visiting environment.
  • Performs home visiting by the NFP Model Elements and NFP Visit-to-Visit guidelines.
  • Apply the nursing process to assess the six prescribed NFP domains, including physical, emotional, social and environmental needs of women and their families as they relate to health and life course development.
  • Assist women and their families in establishing realistic goals and measurable outcomes; utilize reflective practice, stages of change, and other behavioral change approaches.
  • Provide counseling to assist women & their families in attaining targeted goals in areas including prenatal & postpartum care, nutrition, parenting, childcare, family planning, special health problems, housing, continuing education & employment.
  • Evaluate women’s & families’ progress toward target goals, revise the plan of care as appropriate.
  • Develop a working relationship with women and their families that promotes trust and problem solving.
  • Assist clients in developing supportive relationships with family members and friends during pregnancy, birth and childcare.
  • Link women and family with community resources that are relevant to their specific needs.
  • Consult and collaborate with other professionals providing service to women and their families.
  • Meets with NFP Nurse Supervisor weekly for clinical supervision.
  • Participate in weekly 2-hour team meetings and case conferences.
  • Consults and collaborates with other professionals involved in providing services to women and families.
  • Formulates nursing diagnosis based on nursing assessment and client goals.
  • Evaluates client progress toward stated goals and NFP outcomes.
  • Plans home visits by client goals and NFP outcomes.
  • Actively engages in professional development to meet all NFP Nurse Home Visitor competency requirements.
  • Utilizes reflective practice in supervisory sessions.
  • Schedules joint home visits with NFP Nurse Supervisor every four months.
  • Provides information to support staff for timely and accurate data input to the NFP performance management system.
  • Participate in the review and analysis of reports for achievements and areas for improvement.
  • Participates in quality improvement efforts.
  • Schedules joint home visits with MedNorth NFP supervisor every four months.
  • Provides information to support staff for timely and accurate data input into the NSO Data System.
  • Cooperates in review and analysis of the NSO Data System reports for achievements and areas for improvement.

PROFESSIONAL DEVELOPMENT

  • Accurately assesses own learning needs and develops strategies to meet them.
  • Stays informed of current health care developments within the field of maternal/child health, public health, and home visitation.
  • Establishes and records progress toward annual goals as directed.
  • Completes CEU requirements of Licensure.
  • Participate in any NFP required training.

PREFERRED SKILLS:

  • Bilingual preferred
  • Organized and good at multi-tasking
  • Proficient in Microsoft Office, Excel, Outlook.

MINIMUM REQUIRED QUALIFICATIONS

  • Bachelor of Science degree in Nursing from an accredited college or university required
  • Valid NC state RN license required
  • The Registered Nurse license and registration must be maintained for the duration of employment.
  • Current CPR required
  • A minimum of two years of full-time work experience in maternal/child health, pediatric health or a closely related field, in a public health, community-based, hospital, clinic or home visiting setting.
  • Home visiting experience required
  • Excellent written and verbal communication skills
  • Computer skills required
  • Valid NC driver’s license and insured automobile required
    o This license must be maintained for the duration of assignment.
  • Must be able to work a flexible schedule including some evenings
  • Pre-employment checks may include a driver’s license verification and history check, verification of the Registered Nurse License, and a criminal background check.
  • Experience with diverse populations, communities and organizations
