Job Description:
Position Summary
The Nurse-Family Partnership Home Visiting Nurse provides comprehensive, evidence-based nursing services to low-income, first-time mothers and their families from pregnancy until the child’s second birthday. Through regular home visits, the nurse promotes healthy pregnancy outcomes, child health and development, positive parenting practices, family self-sufficiency, and maternal life-course development.
The Home Visiting Nurse establishes a therapeutic relationship with families, conducts assessments, develops individualized care plans, provides health education, and collaborates with community resources to improve maternal and child health outcomes.
Essential Duties and Responsibilities
Client Care and Home Visiting
Conduct regular home visits in accordance with Nurse-Family Partnership model guidelines and visit schedules.
Assess maternal, infant, and family health, safety, and developmental needs.
Develop individualized goals and care plans in partnership with clients.
Provide education related to:
Prenatal care and healthy pregnancy
Labor and delivery preparation
Infant care and child development
Breastfeeding and nutrition
Safe sleep practices
Injury prevention and home safety
Family planning and reproductive health
Maternal mental health and wellness
Life-course development and self-sufficiency
Support positive parent-child interactions and attachment.
Monitor child growth and developmental milestones.
Identify risk factors and implement appropriate interventions.
Care Coordination and Community Collaboration
Coordinate services with healthcare providers, social service agencies, and community partners.
Facilitate referrals and follow-up for medical, behavioral health, and social service needs.
Advocate for clients and families to access resources and services.
Participate in multidisciplinary case reviews and team meetings.
Documentation and Data Management
Maintain accurate, timely, and complete documentation of all client encounters in accordance with agency, state, and NFP requirements.
Complete assessments, care plans, progress notes, and outcome measures within established timelines.
Monitor caseload outcomes and quality indicators.
Ensure compliance with HIPAA and confidentiality requirements.
Professional Development and Quality Improvement
Participate in required NFP education, training, and reflective supervision.
Maintain fidelity to the Nurse-Family Partnership model.
Engage in ongoing quality improvement activities.
Attend team meetings, case conferences, and professional development opportunities.
Minimum Qualifications
Current unrestricted Registered Nurse (RN) license in the state of practice.
Bachelor of Science in Nursing (BSN) required.
Minimum of two years of nursing experience in one or more of the following:
Maternal-child health
Public health
Obstetrics
Pediatrics
Community health
Home health nursing
Valid driver’s license, reliable transportation, and ability to travel throughout assigned service area.
Ability to work independently and manage a community-based caseload.
Preferred Qualifications
Experience working with vulnerable or underserved populations.
Public health nursing experience.
Home visiting experience.
Knowledge of community resources and social determinants of health.
Bilingual skills preferred.

Knowledge of maternal, infant, and early childhood health and development.

Physical Requirements
Ability to travel extensively within assigned counties.
Ability to enter and exit clients’ homes safely.
Ability to lift and carry educational materials and equipment up to 25 pounds.
Ability to sit, stand, walk, and drive for extended periods.
Work Environment
This position is primarily community-based and requires frequent travel to client homes, healthcare facilities, and community agencies. The nurse works independently in a variety of environments while maintaining adherence to agency safety policies and procedures.
