Nurse-Family Partnership
Job Description:
• Ensure all NFP services are implemented with integrity to the NFP Model Elements and comply with funding requirements, agency policies, and contractual obligations.
• Maintain up-to-date knowledge of NFP and NFPx Model Elements, best practices, and innovations.
• Collaborate with the Senior Associate Director on monitoring the NFP contract and budget to ensure proper resource allocation and program sustainability.
• Assign and monitor NHV caseloads, using nursing judgment and data to consider client complexity and overlapping adversities.
• Oversee and ensure the timeliness and accuracy of data entry into NFP and funder-required data systems; use data to monitor program performance, guide improvement, and support staff development.
• Coordinate recruitment and hiring of NHVs to ensure full staffing (8 NHV) and administrative support staff in collaboration with the Senior Associate Director.
• Conduct team interviews with candidates and make hiring recommendations.
• Develop and implement an onboarding process for all new nursing staff; review annually with each nurse.
• Ensure all NFP-required education is completed by NHVs, supervisors, and required administrators, and that new hires have a professional development plan when applicable.
• Provide weekly one-on-one reflective supervision to each NHV, integrating NFP principles to enhance practice, address challenges, and foster professional growth.
• Facilitate case conferences twice monthly to review client situations, strengthen therapeutic relationships, and incorporate multidisciplinary expertise when appropriate.
• Lead team meetings twice monthly for administrative updates, professional development, and team building.
• Conduct joint home visits with each NHV at least every four months, focusing on agreed-upon proficiencies and coding Mastery Assessment and Plans (MAPs).
• Identify and address knowledge or skill gaps through coaching and professional development opportunities.
• Monitor and approve staff timesheets, leave requests, and training participation.
• Lead community outreach and recruitment strategies to maintain a steady intake of eligible clients, including some evening and weekend activities.
• Develop and maintain referral relationships, ensuring timely and accurate referral processing to and from the NFP program.
• Actively recruit and engage diverse stakeholders for the Community Advisory Board (CAB); present program outcomes and updates at quarterly meetings.
• Build and sustain partnerships with health care providers, social service agencies, and community organizations to strengthen program visibility and resources for clients.
• Complete all required reports for NFP and other funders accurately and on time.
• Maintain and ensure availability of program materials, forms, and outreach resources.
• Manage supply requisitions to ensure NHVs have necessary materials for client visits.
• Participate in agency-wide initiatives, community events, and relevant professional networks.
• Attend meetings and data review sessions as required by funders to maintain program compliance.
• Perform other related duties as assigned.
